FBC Wellness Survey for Men
Men of First Baptist Church:
The purpose of this survey is to determine the overall "health" of the men
in our church so that the church leadership and ministries may better plan wellness
activities and information on your behalf.
The survey also provides information as you answer each question. Please take
the time to response. It will take you less than three minutes and is anonymous.

(TO THINK ABOUT: Most illnesses are easiest to treat when identified early;
screenings such as cholesterol, skin, hearing or, for adult men, a prostate
specific antigen (PSA) blood test can have an important affect on your health.
According to American Health Association –
“Heart disease is the leading cause of death for men. Smoking, high blood pressure,
high cholesterol, family history and age are the biggest factors that contribute
to heart disease.”)

4. Within the last 6 months, how often have you stopped by the FBC Health & Wellness BP Center for a blood pressure screening?
(TO THINK ABOUT: High blood pressure or hypertension can be life-threatening if left
untreated. Blood pressure is the force of blood against the artery; Systolic
Pressure(SP) is the highest pressure in an artery when your heart is
pumping blood to your body; Diastolic Pressure(DP) is the lowest pressure in an
artery when your heart is at rest. BP measurement is SP/DP and is normally shown
120/80. Stop by for a free screening every 2nd Sunday or 3rd Sunday when there’s a
5th Sunday in the month! )
